The BNPCa Chronic Disease Collaborative used it to reflect on the experience of clients accessing chronic condition programs. The findings were used to inform our planning and reinforced our work in providing information about availalbe services, the need for effective feedback tools for GPs, capacity building in assessment, referral, care planning and self management support and improving organisational structures that support self managment and service coordination.
