| How is this document used | The Hume MOreland Primary Care Partnership Integrated CDM committee identified that consulting people who are living with chronic disease in the area was also required to inform the development of a shared vision of chronic illness across our catchment. The Patient Assessment of Chronic Illness Care was agreed to be administered in focus group-style community conversations throughout the area, in order to validate agency responses to the ACIC as well as contribute the perspectives of people living with chronic disease towards the shared vision.
